  • Intelligence, a prepared statement of what changed and why it matters.
  • Evidence, the specific records a statement rests on. No evidence, no claim.
  • Decision, a recorded choice with a rationale and an owner.
  • Action, the work that follows a decision, carried as its own record.
  • Outcome, what actually happened afterwards, assessed against the decision.

These are linked, not separate systems: a decision points to its evidence, work points to the decision that created it, and an outcome points back to the decision it judges.
